Same Day Rideshare & Uber Medical Certificates
If you operate rideshare passenger vehicles or drive with Uber in Queensland, you must complete a commercial driver medical assessment as part of the requirements through the Department of Transport and Main Roads.
At Narangba Station Health Centre, we provide same-day medical assessments for rideshare and Uber drivers with all testing completing on-site. Within approximately 30 minutes, our doctors will complete the relevant medical assessments and documentation required for your application or renewal whilst offering health and medical advice as appropriate.
To obtain your medical certificate, please book an appointment online or call reception on (07) 3521 5523. Please bring your relevant driver medical form and any supporting documentation with you to your appointment.
Required Forms
All patients should print and complete Part 1 of Form F3712 and Form F3195 prior to attending your appointment. Please also bring the additional form that applies to your situation:
- New Applicants: Form F2978
- Renewals: Form F2979
What to expect?
Your Uber driver medical is designed to be straightforward and efficient. During your appointment, our doctor will review your health and complete the checks required to assess your fitness to drive. With testing available on-site, most appointments can be completed in around 30 minutes.
1. Health Review
Your doctor will review your medical history, current health and any conditions or medications that may affect your ability to drive safely.
2. Vision Check
Your eyesight will be assessed against the relevant commercial driving standards, including your visual acuity and other vision requirements where applicable.
3. Physical Review
We’ll assess areas such as mobility, movement and coordination where required to ensure you can safely operate a vehicle.
4. Form Completion
Once you are deemed medically fit, your doctor will finalise the relevant medical documentation for your Uber driver authorisation application or renewal.
Fees & Medicare
For both new and existing patients, Uber driver medical assessments cost $198 (inc GST) excluding any treatment room fees or additional tests required.
For more details on pricing, please visit our Fees page.
Medicare Rebates:
All workplace medical assessments are privately billed. Please note that there is no Medicare rebate for this consultation. We recommend that you retain a copy of receipt and, if the cost incurred is classed as a business expense, you may be able to claim with your tax return.
What to bring?
To help your appointment run smoothly, please bring:
- All required medical forms (See here).
- Your current driver licence or photo ID.
- Glasses or contact lenses if you use them.
- Hearing aids if you normally wear them.
- A list of your current medications.
- Any relevant medical reports or results.
No Printer?
If you do not have access to a printer or have issues with downloading the forms, please arrive 15 minutes early to your appointment and our reception team will be able to provide you with a copy to complete.
How It Works?
To complete your drivers medical assessment, follow these simple steps:
- Step 1: Book an appointment with one of our friendly doctors.
- Step 2: Print blank copies of both the F3712 and F3195 forms.
- Step 3: Complete part 1 of both documents prior to your booking.
- Step 4: Bring both forms with you to your appointment.
- Step 5: Your doctor will then complete your health assessment.
Once you have a signed copy of both the F3712 and F3195 from your medical assessment, you will then need to complete a Driver Authorisation Form. Please also print a copy of the relevant form:
- First Time Uber Applicants: Download & Complete Form – F2978
- Renewing Uber Applicants: Download & Complete Form – F2979
You can then submit your application to your closest Department of Transport and Main Roads. It typically takes 2-3 weeks for this to be approved and it’s a good idea to keep a copy of these forms for yourself as Uber will also require a copy. Once approved, upload your drivers authorisation to your Uber account. This should have you on your way to becoming an Uber driver and generating income.

Rideshare Medical FAQs
Do you still have questions about standard commercial driving medical checks for Uber and rideshare vehicles? No problem! Here are the most commonly asked questions our GP’s receive from new and renewing rideshare and Uber drivers in Brisbane.
Do you accept referrals?
Yes, we accept GP referrals for commercial driving medical assessments. However, referrals are not required and both new and existing patients can book directly.
How long do assessments take?
On average, your driving medical for Uber will take between 15-20 minutes to complete. While we strive to ensure that your appointment runs on time, if you have not completed part one of your certificate or are unable to arrive on time, the length of time it takes to complete your medical assessment may increase.
Can I obtain a Medicare rebate?
Unfortunately, Medicare does not cover the cost of medical assessments required by private employers, including Uber. These assessments are considered occupational health services, which fall outside of Medicare's coverage. However, some private health insurance plans might offer partial reimbursement for such expenses and so it's worthwhile consulting with your health fund.
What forms do I need?
To complete your Uber medical, you will need the following forms from the Department of Transport and Main Roads:
- F3712 - Medical Certificate for Motor Vehicle Driver
- F3195 - Private and Commercial Vehicle Driver's Health Assessment
- F2978 - Driver Authorisation Application
- F2979 - Driver Authorisation Renewal Application
Depending on whether you are a new Uber driver or renewing your drivers authorisation will depend on whether you need the F2978 or F2979 form. You will not need both, just one or the other depending on your situation.
What happens if there is a medical concern?
Your doctor will explain the concern and what happens next. Depending on the issue, they may request further information, arrange additional testing, recommend specialist review or, if applicable, complete your medical certificate with relevant conditions or restrictions.
Can I get my certificate on the same day?
Both same-day appointments and same-day certificates are available depending on doctor availability. Please book online or call reception on (07) 3521 5523 and let the team know you require a commercial driver medical so the correct appointment type can be arranged.
How often do I need to renew medicals?
This depends on your licence or driver authorisation, age, medical history and the expiry date stated on your medical certificate. In Queensland, rideshare drivers under 75 must renew their commercial medical certificate every five years, while drivers aged 75 and older must renew annually.
