How It Works
To complete your commercial drivers medical assessment, please follow the steps below:
- Step 1: Book an appointment with one of our friendly doctors.
- Step 2: Print a copy of the F3712 and F3195 commercial drivers form.
- Step 3: Print a copy of the F2978 (New) or F2979 (Renewals) form.
- Step 3: On both forms, complete Part 1 prior to your appointment.
- Step 4: Read all forms carefully prior to your appointment.
- Step 5: Attend your appointment 10 minutes before schedule.
- Step 6: Your doctor will then perform a series of assessments as required by law.
After completing the F3195 Private and Commercial Vehicle Driver’s Health Assessment, your doctor will then complete the Medical Certificate for Motor Vehicle Driver (form F3712) for you to present to the Department of Transport and Main Roads. In addition to the F3712 and F3195 forms, you will also need to complete your Driver Authorisation Form.
It is recommended that you complete the health questionnaire prior to attending your appointment and if you need to wear glasses, contact lenses or hearing aids when driving, bring these with you to the assessment.
Please Note: If you do not have access to a printer or have issues with downloading the medical form, please arrive 15-20 minutes early to your appointment and our reception team will be able to provide you with a copy for you to complete prior to your booking.
What To Expect?
Our commercial driving medical assessments are designed to be thorough yet straightforward. We ensure that you can get back on the road with confidence while complying with the law.
Your commercial driving medical assessment should take approximately 15-20 minutes to complete and you can expect the following assessments:
1. Health Review
We conduct a comprehensive review of your overall health, focusing on chronic conditions such as diabetes, heart disease, and arthritis that might affect your driving abilities.
2. Vision Test
Our vision test ensures that your eyesight meets the required standards for driving. This includes tests for visual acuity, peripheral vision, and depth perception.
3. Mobility Check
We assess your physical ability to operate a vehicle, including your range of motion, strength, and coordination. This ensures you can comfortably and safely control your vehicle.
4. Cognitive Evaluation
Truck Drivers Medical Assessment
One of the most common commercial fitness to drive medicals we complete is for truck drivers in Queensland. As a heavy good vehicle driver, the demand on your body is greater than that of a private vehicle driver. You are on the roads longer and the complexities of your vehicle require good vision, physical ability and cognitive capacity.
For licensing purposes, the commercial vehicle driver standards apply to drivers of medium rigid vehicles and larger. Of which, the specific requirements for medical assessments for commercial licensing (whether assessments are required and how often) differ between the states and territories.
To find out more about fitness to drive standards for truck drivers, visit the commercial drivers page on the official Austroads website.
Taxi Drivers Medical Assessment
Similarly to truck drivers, public passenger vehicle licence holders such as taxi drivers, Uber rideshare drivers and restricted hire car drivers are also required to undergo commercial fitness-to-drive medical assessments, given the unique demands of their profession.
As a passenger vehicle driver, not only are you driving for long periods of time, sharp cognitive abilities are also paramount to ensure passenger safety and comfort. The medical assessments ensure that drivers meet the necessary health criteria to safely operate their vehicles.
To book a drivers medical for public passenger vehicle drivers, simply call us on (07) 3521 5523, or book online via our online booking system. Alternatively, you can download the AMS Connect App to book an appointment directly from your phone.

How long is a commercial driver medical certificate valid?
A commercial driver medical certificate will remain current for up to 5 years from the date it was issued by the medical practitioner.
However, a conditional medical certificate may be issued by the medical practitioner for a lesser period, especially if you suffer from a medical condition that requires more regular assessment.
Driver authorisation cannot be issued past the expiry date of a medical certificate.
